Miscellaneous

  • Company: "A+" rated, exclusive, preferred market, in-house underwriting and claims authority

  • Financing available: terms vary by state

  • Preferred Program for Preferred Accounts with competitive premiums

  • Territory: Most states except FL, HI, AK

The following mandatory items must be completed in order to receive a quote:

          Workers' Compensation Underwriting Questionnaire

(All applications can be downloaded and filled out on your computer or printed and filled out by hand.  You will need to download Adobe Reader in order to view the applications Download Adobe Acrobat Reader Software)

 

2. ACORD Applications (Umbrella, Auto, Crime, Inland Marine)

3. Financial Statements (Income, Balance Sheet, Cash Flow)

4. Copy of Last Inspection by Department of Human/Health Services

5. Copy of Company Loss Reports

6. Resumes for Administrators, DONs, Executive Directors

7. Care Plan for Any Acquired Stage III or Stage IV Pressure Sores

8. Copy of Licenses

9. Company Brochures/Web Site
